Heart Worms

Had a dog test positive for heart worms. I was told to give half a CC of IVOMEC. But the dog is 40lbs and that says .1cc IVOMEC. I read threads but I just need the bottom line. So how much? How often? How long do I have to keep her in dog prison? I really like the dog and want her to be around as long as possible. I just do not have the money for a vet.
